Start with the recipient, not a standard bundle
There is no default quantity in this organizer. Begin with the office that will receive a document and ask what it actually needs. Create a separate request for each recipient and document combination. That lets you distinguish a consular submission from a receiving provider’s file or another institution’s request without assuming they accept the same format.
Enter a quantity only when you have a number to record. Leave it blank when the answer is unknown, and enter zero only when you mean that no copies of that type are requested. Mark whether the recipient has confirmed the information. These entries are your planning notes; the tool does not verify them with an office.

Use public office names and document types. Leave out personal details. Leave unknown quantities blank; zero means no copies of that type are requested.

Keep certified records and ordinary copies separate
A certified death certificate and an ordinary photocopy should not be counted as interchangeable items. Texas DSHS distinguishes certified death certificates from death-verification letters and says a verification letter is not a legal substitute for a certified certificate. Ask the recipient for the exact record type before ordering.
The two quantity columns stay separate in the worksheet. The ordinary-copy column can record photocopies requested for a file; it does not certify them. If an office says “original,” ask whether it means an original certified copy issued by the registrar, another original document or something else. Put that unresolved question in the authentication note instead of guessing.
Use the proper records request process
Texas DSHS identifies who may request certified vital records and explains the documentation other applicants may need to establish a direct, tangible interest. Being the person who maintains this worksheet does not itself establish a right to order a record. Check the official application requirements or ask the registrar which applicant should submit the request.
This page does not ask you to prove that relationship or enter identification numbers. Keep identity documents and the actual certificate in the issuer’s designated process, not in the worksheet. A short business label such as “consular office” is enough to identify a request here.
Record authentication as a separate question
The Texas Secretary of State explains its universal Apostille certificate: it serves as an apostille for Convention destinations and as an authentication for other destinations. The Secretary also identifies possible additional federal authentication for non-Convention use. The destination and document determine what must be confirmed; ordering more certified copies does not answer that question.
Under the Apostille Convention, the certification concerns the signature, the signer’s capacity and, where appropriate, the seal or stamp. It is not a transport reservation. Ask the intended recipient whether authentication is required for this particular use and what additional steps apply. Record the answer as a note, not as a government decision generated by this tool.
Read the subtotal with its unresolved items
The worksheet adds the quantities you enter. It displays certified copies for recipients, your own optional certified spare quantity, their combined subtotal and ordinary copies separately. A blank quantity is excluded from the sum and counted as unspecified. A request marked awaiting confirmation remains visibly pending even if you entered a quantity.

Do not treat the subtotal as a purchase instruction while any relevant request is unresolved. The tool does not subtract documents that an office might return or assume one copy can serve several offices. Confirm reuse, return arrangements and the sequence directly before changing your quantities.
Keep the worksheet current
Use Add request when another office or document needs its own row. After calculating, review the text before copying or downloading it. Edits clear the previous result, so calculate again after any change. Clearing the page does not delete copies you already downloaded. A text download can be opened and printed using your own device.
The organizer code does not send entries or save them in browser storage. Existing site services have separate privacy terms. Use public office names and document categories; do not enter a deceased person’s name, certificate number, medical details or payment information. The worksheet neither orders records nor contacts any recipient.
Questions about copies and authentication
- Does the certified subtotal tell me how many copies I must buy?
- No. It adds your entries and optional spares. Confirm every relevant recipient’s instructions before ordering.
- Why does an empty quantity differ from zero?
- An empty field means the quantity is unknown. Zero means you entered that no copies of that type are requested.
- Does a confirmed row mean the government has approved my documents?
- No. It records your statement that you checked the recipient’s instructions. The tool does not obtain or verify an approval.
